Intervention Form
Interventions should be reported in an official capacity through the method agreed by your organisation via the Safety Management and Information System (SMIS) or your organisations safety app.
However if your organisation does not have a mechanism to record interventions, or you’d just like to make Samaritans aware of your intervention, you can complete the form below and send it to [email protected].
